是的. 我的融合门户 has security measures within it that prevent unauthorized users from reading communications about your health. This information is accessible to read through the correct username and password to log in to your portal account.

There are two options to sign up for 我的融合门户. At your next scheduled visit you can provide your email address to a 澳门新葡新京 team member. They will be able to send you a secure email invitation to sign up for 我的融合门户. Through this email invitation you are able to create an account. This invitation to join expires after 90 days.If you do not have a scheduled appointment and wish to sign up for MY 融合 Portal, you can contact the 医疗记录 Department by calling 740-687-8053, 选项4.The hours of operation are Monday- Friday, 7 a.m.-8:30 p.m. The staff members in the 医疗记录 department will be able to walk you through the process to sign up to the portal.

Online self-enrollment is available if you are 18 and over. You will need your medical record number in order to successfully register online. Online self-enrollment is currently not available for caregivers or guardians. 点击这里进行网上自助登记.

病人 12 and under must have a proxy to access their records via 我的融合门户. Authorized Proxy access will be automatically revoked when the patient turns 13 years old, 直到17岁. 病人 13-17 may have their own portal account. 病人 age 18 and older may grant proxy access to their portal account through an approved Personal Representative Authorization Form, which may be done at any 澳门新葡新京 or Fairfield Healthcare Professional Location.

Test results may take up to two weeks to process. Once your results are complete and finalized they will show up in your account.

We now offer the ability for you to request a secure connection from your 我的融合门户 health record to some of the health management apps you may use.

This new process provides access to your health and wellness data in an app of your choosing that will enable you to make informed healthcare decisions. Once we receive your app request, 澳门新葡新京 will review and evaluate the request. You will be contacted if there is a reasonable need for the requested app with next steps to connect your health records to your apps. There may be a fee associated with this process.
